becton dickinson facscalibur
the bd facscalibur™ flow cytometer is a versatile tool with enormous potential for the study of cells and particles.
no other laboratory instrument provides multiparametric analysis at the single cell level, and the flow
cytometer or application-variants of the flow cytometer will become more valuable as medical diagnosis and
therapy changes. a flow cytometer can discriminate cells at speeds up to 70,000cells/second. in addition to
a high speed processing, this instrument offers high fluorescence sensitivity and minimal sample contamination.
the bd facscalibur system can perform a wide range of complex tasks such as immunophenotyping,
which is critical to the initial diagnosis and classification of the acute leukaemias, chronic lymphoproliferative
diseases, and malignant lymphomas. in addition, immunophenotyping provides prognostic information not
available by other techniques, hence provides a sensitive means to monitor the progress of patients after
chemotherapy or bone marrow transplantation, and often permits the detection of minimal residual disease.
all these tasks are performed at biolab in addition to bone marrow transplantation and stem cell analysis,
hiv infection (cd4/cd8) and many applications in research. this technique is used not only for complex tests

cobas taqman 48
the cobas taqman 48 analyzer is a real-time pcr analyzer that provides unmatched
accuracy, flexibility, maximum efficiency, a superior dynamic range
and contamination risk elimination. this method is mainly utilized for the monitoring
and quantification of target copy numbers of viral load such as hepatitis
b and c viruses. cobas taqman 48 is one of the few worldwide fda approved
instruments.
corbett rotor-gene 6000
real-time polymerase chain reaction (pcr) is the ability to monitor the
progress of the pcr as it occurs. it enables the detection of very small amounts
of specific nucleic acid sequences during the early phases of the reaction
and on a real-time basis. data analysis, including standard curve generation
and copy number calculation, is performed automatically. the rotor-gene is
an innovative real-time cycler that enables high-precision real-time pcr. this
instrument is usually utilized for the detection of infectious diseases.
gene sequencer abi prism – 310 genetic analyzer
dna sequencing by capillary electrophoresis is the ideal alternative to gelbased
dna analysis for any research, clinical or forensic lab. the abi prism®
310 genetic analyzer is an automated single-capillary genetic analyzer designed
for a wide range of sequencing and fragment analysis applications.
it performs comparative sequencing, linkage analysis, snp detection, discovery
and validation, mutation detection, and many other applications.
it allows rapid diagnosis of different diseases. it enables to automate applications
from de novo sequencing to snp validation and linkage mapping.
examples of such applications are amnio pcr for aneuploidy screening
of chromosomes 13, 18, 21, x and y, and duchenne and becker muscular
dystrophy.

it is a superior automated hormonal immunology analyzer,

amplification.
cobas (integra 400 plus)
cobas (integra 400 plus) is a sensitive and delicate automated
clinical chemistry analyzer that performs more than 140 applications

measuring technologies – absorbance photometry, turbidimetry,
fluorescence polarization and ion selective potentiometry. it represents

the bio-rad d-10 comprehensive, fully automated hemoglobin
testing system is a bench top high-performance
liquid chromatography (hplc) system dedicated to the
quantification of hba1c, hba2 and hbf in whole blood.
combining diabetes and ?-thalassemia testing on a single
platform, the system offers simple, efficient operation, developed
for flexibility and reliability. whole blood primary
tube sampling eliminates off-line sample preparation.

test procedure
all tests are performed according to written procedures, and updatingd in line with technical developments.
the preparation, set up and analytical and pathological validation of tests are subject to written procedures.
the internal quality control for each test is subject to a detailed description in terms of control samples, testing
conditions, acceptability guidelines and the anomaly procedure. biolab takes part in various external control
programs so as to validate the methods used and detect any anomalies.
the request form
clear and legible completion of the request form is an essential requirement for acceptance of samples. the
following details are required.
patient details
• full name
• hospital/lab/clinic reference number
• date of birth
• sex
referral details
• full name of referring clinician (no initials please)
• contact telephone number of referring clinician
• tests required
sample details
• type of sample
• date and time of sample obtained and collected
clinical details
• appropriate application of tests is entirely dependent upon the information we receive from you.
please provide us with all relevant clinical details, family history and results of any previous genetic testing
that may have been carried out (with name of laboratory and reference numbers if possible).
• reasons for referral.
• it is strongly recommended that when taking a blood sample, the patient is asked to list the medications
currently used (if applicable). this information is crucial, as some medications may inhibit or interfere
with some of the tests performed.
sample handling
• put the sample in the appropriate container tube (refer to next section).
• label with patient name and patient reference number.
• place in a sealed, leak-proof sample bag.
• place the sample bag in a rigid container (isothermal shipping box).
• the request form should be put inside the box.
• transport to the laboratory as soon as possible, preferably on the day of sampling.
• frozen samples must be transferred at -20°c with the appropriate amount of dry ice (not less than 4kgs).
• refrigerated samples must be transferred at 4°c (placed on top of cold cells).
• ambient samples must be transferred at room temperature (do not freeze or expose to heat).
prior arrangement is required before and during holidays and long weekends, as delay may influence cellular
viability.
practical information

urgent samples
• prenatal and neonatal samples are always treated as urgent by the laboratory. for other urgent samples,
please state ‘urgent’ clearly on the request form.
high risk samples
• samples from high-risk patients (e.g. carriers of hiv, hepatitis b, and hepatitis c) must be labeled with an
appropriate hazard warning sticker. the nature of the risk must be clearly stated on the request form.
• high risk samples requiring containment level 3 facilities, e.g. known cases of sars, nvcjd etc. cannot be
processed.
sample rejection
samples arriving in a totally unsuitable condition are unlikely to produce satisfactory results, therefore are
not processed, and a repeat sample will be requested, e.g.:
• sample in wrong tube or wrong temperature
• sample delayed in transit for more than the maximum transit stated
• sample clotted
• non-viable sample (lyzed, infected, exposed to a fixative, dried, exposed to high temperature)
• broken sample tube
• spilled samples
• sample quantity not sufficient for testing (see table 2)
• inadequate labeling
- the referral request form and sample must be labeled with the name and details of the patient to be
tested (not those of another family member such as the wife or mother).
- inadequate labeling of a sample (e.g. discrepancy between name on sample and name on referral
request form or no name on sample or no name on r equest form) may result in a sample not being
processed.
- a minimum of two identifiers should be present on the sample tube (e.g. patient’s name, your lab/clinic/
hospital reference number)
patient consent form
informed patient consent is required for all genetic tests from all patients. the laboratory assumes that provision
of clinical sample, and a signed referral form, implies that consent form provided has been signed.
results
• results are validated and, thus released online (www.biolab.jo) as soon as the test is completed and the
results have been authorized by qualified staff.
• telephone reporting of results is discouraged due to the risk of error in verbal transmission.
• requests for results to be faxed or e-mailed may be made by telephoning, faxing or emailing the referral
department (refer to page 3).

introduction
in recent years, the field of medical genetics has been expanding our knowledge in
the involvement of genes and chromosomes with certain diseases and disorders. it is
now well established that genetic aspects are responsible for a variety of birth defects,
chronic diseases and a high percentage of mental retardation.
the molecular genetics laboratory at biolab provides comprehensive dna-based diagnostic

cytogenetic techniques allow for the unambiguous identification of each human chromosome
and the detection of aneuploidy and many large structural rearrangements that
cause physical and / or mental retardation, congenital anomalies related to inherited conditions,
abnormal sexual development, and other defects. new techniques allow for increased
resolution of chromosome banding patterns, permitting differentiation of a greater
number of abnormalities. fluorescent in situ hybridization (fish) technique (using chromosome-
gene- or translocation- specific dna probes) is used in the detection of genetically
important trisomies, diagnosis and classification of various neoplasia and hematological
malignancies as well as prenatal and pre-implantation diagnosis.
the department’s activities fall into three main sectors, namely prenatal diagnosis (amniotic

small subtle chromosome abnormalities
• cytogenetic analysis relies on g-band quality and resolution. in general, blood samples give the best
quality chromosomes and therefore provide the best chance of detecting small subtle chromosome
abnor malities. chromosomes from other tissues (e.g. amniotic fluid, chorionic villus and pr oducts of
conception) give poorer quality chromosomes hence the risk of missing a subtle abnormality is
increased.
• it should be noted that on rare occasions a subtle abnormality may be missed at prenatal diagnosis,
only to be diagnosed later on a postnatal blood sample.
• it should also be understood that even a g-band blood karyotype can never exclude extremely subtle
chromosome abnormalities that are at the limit of resolution of light microscopy.

detection of mosaicism
• it is well established that although mosaicism may be detected by routine karyotyping it can never be
100% excluded. however, if there is an indication of suspected mosaicism, additional cells will be examined
to exclude 10% mosaicism at a 95% confidence level.
interpretation of mosaicism in prenatal diagnosis
• true mosaicism, when detected prenatally, can be difficult to interpret and a further invasive diagnostic
test may be required. mosaic cell lines may be unevenly distributed between the fetus and extra-fetal
tissues leading to false positive and false negative results in the most extreme cases. ‘confined placental
mosaicism’ (cpm) is observed in approximately 1-2% of cvs samples.
• pseudomosaicism can arise as an artifact of culture and is not representative of the fetal karyotype.
this is normally present in only one of two or three independently established cultures and can therefore
be interpreted accordingly. in most cases, no further invasive testing is required.
maternal cell contamination in prenatal diagnosis
• maternal cell contamination of chorionic villus and amniotic fluid occurs in approximately 1/250 samples
and may occasionally complicate the interpretation of results.
normal variation
• each chromosome pair has a specific and identical g-banding pattern in all individuals. however, variation
may occur around the centromeric regions and short arms of some chromosomes that is of no clinical
significance. these variations are known as ‘polymorphic variants’, ‘polymorphisms’ or ‘normal variants’.
• in a few cases, it is necessary to distinguish between such variation and a true abnormality. it may therefore
be necessary to karyotype the parents or to carry out further tests on a repeat sample.

sperm wash (iui)
sperm washing is a procedure used to prepare sperm for use in intrauterine insemination (iui), which is the
placement of sperm in the uterus for the purpose of enhancing the chance for pregnancy. preparation for an
iui involves separation of the sperm from the seminal fluid. the semen sample must be washed free of debris,
white blood cells, and prostaglandins, which can cause the uterus to contract. the processing also removes
dead sperm and concentrates the sperm into a small volume that can easily be handled by the uterus. this
procedure is usually done in cases of unexplained infertility, male-factor infertility, women with endometriosis,
men with antisperm antibodies, or men that are hiv positive.

indications for autoimmune testing
the cause of autoimmune diseases is unknown, but it appears that there is an inherited predisposition to develop
autoimmune disease in many cases. in a few types of autoimmune disease (such as rheumatic fever),
a bacteria or virus triggers an immune response, and the antibodies or t-cells attack normal cells because
they have some part of their structure that resembles a part of the structure of the infecting microorganism.
autoantibodies may be present in healthy individuals and may also occur transiently with intercurrent illness
or may be induced by drug therapy.
limitations of autoimmune testing
the interpretation of all autoantibody tests is highly dependent on the likelihood of disease in the patient. the
results should always be interpreted with the clinical features of the patient and never in isolation. autoantibodies
may be present in healthy individuals and may also occur transiently with intercurrent illness or may be
induced by drug therapy. conversely, autoimmune disease may be present in the absence of detectable
autoantibodies. do not use these tests as ‘screens’ for autoimmune disease but rather decide the clinical
diagnosis and the likelihood of autoimmune disease, then use specific autoantibody tests as diagnostic aids.
on rare instances, technical limitations associated with some of the technologies used may be present.

the incidence of autoimmune diseases is increasing worldwide and it represents the third
cause of morbidity in developed countries (after cardiovascular disease and cancer). the
search for better management of these diseases and even for a cure is being pursued by
scientists from many disciplines. tools from the areas of genetics, neuroscience, cell biology,
virology, and infectious disease are being applied to the problem and are yielding

any molecule in the body may act as an auto-antigen and induce an autoimmune reaction,
meaning that the range of possible auto-antibodies to be sought is substantial. autoimmune
disorders fall into two general types: those that damage many organs (‘systemic’),
and those where only a single organ or tissue is directly damaged by the autoimmune
process (‘localized’). in some pathological situations, auto-antibodies are extremely sensitive
markers which can be detected months or even years before the onset of symptoms.

some that are rare or were only recently characterized. auto-antibodies can be
detected either by indirect immunofluorescence on cell smears (transfected hep2 cells,

serological tests can be used to screen for or diagnose viral, fungal and parasitic infectious diseases, and
assess the subject’s immune status which is of particular importance to pregnant women and the immunodeficient.
the serologic indirect techniques by quantification of antibodies have extremely been useful in the diagnosis
and the therapeutic or vaccination follow-up.
serological tests are performed to detect current, active, or past infections in people who are at risk for reactivation
of infection (such as organ transplant recipients and people with a suppressed immune system).
the presence of antibodies to certain infectious diseases indicates a current or past infection. if the number of
antibodies rises over a few weeks, it may mean that one has an acute infection. chronic infection (in which
the antibody count stays about the same over time) can reactivate in a person with a suppressed immune
system.
limitations of serology testing
the results require in the majority of the cases to be compared on two subsequent serum samples, to observe
a possible increase in antibodies level. in addition, the infectious serology results may not be considered as
the only element of final diagnosis. in all cases, they have to be interpreted and challenged against the
clinical context. results may reveal hard to interpret, especially when the scope is to specify the time of the
beginning of an infection.

cervical cytology, one of the department’s fields of expertise, has been perhaps the most

complexity of findings and the cytologist’s experience and state of mind.
limitations for fine needle aspiration
though this procedure allows a minimally invasive, rapid diagnosis of tissue but does not preserve its histological
architecture. in some cases this limits the ability to make a definitive diagnosis. as with any invasive
procedure there are risks, and as with all diagnostic tests involving sampling and interpretation, important
diagnoses can be missed.

false-negative test results generally are attributed to inability to sample some lesions (eg, voided urine
