Submental & Submandibular triangles
Curricular Objectives
By the end of this session students are expected to be able to:
Practical
1. Recall the boundaries of anterior triangle and its 4 subdivisions
2. Locate the Submental triangle and its bounding structures
3. Identify the Mylohyoid muscle and the bones it attaches
4. Distinguish the structures bounding the Submandibular triangle and its location
5. Recognize the Submandibular salivary gland and the structures in its vicinity
Theory
1. Outline the subdivisions of the anterior triangle
2. Describe the boundaries, roof, floor and content of the sub mental triangle
3. List the areas drained by sub mental lymph nodes
4. Describe the boundaries, roof, floor and content of the sub mandibular triangle
5. Recall the location of Platysma muscle in relation to the Submandibular triangle
6. Discuss the relations of the Submandibular salivary gland
7. Summarize the course of facial artery and vein in relation to Submandibular gland
Check List
? Key landmarks
? Body of mandible (lower border)
? Minor task: hold the mandible in your hand and locate the Symphysis menti
? Hyoid bone
? Try to feel it in your neck
? Mastoid process
? Mylohyoid Muscle
? Right and left muscles are flat, unite at midline to form the floor of the oral cavity
? Originate from the medial (inner) side of the body of mandible to be inserted at
…………….. and …………………..
? Actions of this muscle include ……………………….
? Digastrics muscle
? Anterior belly: origin…………..….….. ? intermediate tendon (mandibular nerve)
? Posterior belly: origin……………….. ? intermediate tendon (facial nerve)
? Intermediate tendon: anchored to ……………….. by a fascial sling
? Stylohyoid muscle
? Run along the upper border of ……………………muscle
? Originate from ………………………. Run along the upper border of ………………….. to be
Inserted into…………………………….. by two slips on either sides of intermediate tendon
of digastrics muscle

? Hyoglossus muscle
? Key muscle of the Submandibular region
? Flat muscle extending from the hyoid bone below to the side of the tongue above
? Its superficial and deep relations are very important to surgeons
? Important Superficial relations include:
1. ……………………….
2. ……………………….
3. ……………………….
A- Submental triangle
? Boundaries:
? Digastric muscle (anterior belly)
? Hyoid bone (body)
? Midline
? Floor: Mylohyoid Muscle
? Roof: Investing layer of deep cervical fascia
? Content:
1. Submental lymph nodes (3-4, belong to superficial cervical group of LN)
? The area drained by these lymph nodes include
? ……………..
? ……………..
? ……………..
2. Commencement of anterior jugular vein ??which drain to……………….??
Note: The contents lie within superficial fascia of the Submental triangle
? Comprehension question
An enlarged lymph node just below the chin may indicate one or more of the followings:
a. Tumor of the posterior one third of the tongue
b. Tumor of the anterior part of the floor of the mouth
c. Boil of the chin

B- Submandibular (Digastric) triangle
? Boundaries:
? Body of mandible
? Digastric muscle (anterior belly)
? Digastric muscle (posterior belly)
? Roof: Investing layer of deep cervical fascia (covered by: superficial fascia, Platysma, skin)
? Floor: Mylohyoid and Hyoglossus muscles
? Content:
1. Submandibular salivary gland (most prominent structure)
2. Submandibular lymph nodes. These nodes drain a wide area. Give some examples of
area drained by these lymph nodes
3. Facial artery ??passes ……….. to the gland??
4. Facial vein ????passes ……….. to the gland??
5. Hypoglossal nerve (12th cranial nerve which is responsible for …………………..??
6. Others: lie at the posterior part of the triangle include
Carotid sheath and its content, external carotid artery
? Comprehension question
A 30 years old female was putting her makeup when she accidently felt a mass at the
Submandibular triangle.
1. List the two most common causes of this swelling.
2. The character of pain may help to differentiate between the two, how?
? Home work:
1. An important nerve may pass along the roof the Submandibular triangle. Name it
2. Where the Submandibular fossa is located?
