Community Health Nursing Philosophy: 

Community Health Nurse Practice:

Community Health Nurse Practice:

* Is focussed on wellness not sickness.
* Is focussed on prevention not just treatment of problems.
* Is focussed on assisting people and communities make their own decisions regarding health care.
* Is focussed on assisting those with existing health conditions to maximise their potential and prevent deterioration if possible.
* Is based in the community in a range of settings, wherever there is a need for support, information and education.
* Responds to public health needs in relation to communicable diseases.
* Has the whole community as their client, as well as groups, families and individuals.
* Recognises the impact of life course and social determinants of health and works in partnership with the community to address these.
* Utilises the primary health care philosophy to guide practice.

* Has a specialised body of knowledge which reflects current research and evidence based practice?
* Community health nurses recognise health as “ a state of complete physical, mental and social well-being and not merely the absence of disease and infirmity”  W.H.O.
* Community health nurses deal with clients in a holistic manner, working with them wherever they are on the health – illness continuum.

What guides the practice?

* We recognise we must identify the needs of the community as well as individuals and groups, as a healthy community supports and empowers individuals to develop and maintain life long healthy lifestyles and choices.
* Many health issues require a community wide approach and a community involvement in solving them.
* Communities are unique and dynamic with a variety of norms, cultures and value systems.
* Community health nurses are creative and adapt their practice to respond to the needs of each community and groups and individual clients who are members of that community.
* Communities need continuous monitoring of the overall health status of the community as well as responses to episodic health needs.
What is different about the practice?
* Community health nurse practice, by the nature of the work setting, requires autonomy, self-direction and use of a high level of professional judgement.
* Community health nurses interact with a wide range of both professional and non professional people within the community and must have a variety of interpersonal skills.
 Community Health nurse role is:
* To promote health and health behaviour in the community

* To act as a health resource person for the community

* To identify health issues which may impact on the well-being of individuals, families, groups and communities?

* To refer identified health issues to appropriate agencies and ensure co-ordination of care occurs

How to go about this role:

* Undertaking a detailed community health assessment to identify health issues, social determinants and life course determinants of health for this community.

*     Utilising a community development approach to improve the social capital of the community and acknowledge their role in bringing about change.

* Health promotion and health education with individuals, families, groups and a defined community.

* Health assessment of individuals, families and groups in the community.
* Providing information and resources on health issues.
* Working in partnership with clients to enable and empower them to maintain and improve their health and wellbeing.
* Working as an advocate for clients to ensure their voices are heard and their views recognised. 


Characteristics of Community Health Nursing:

* It is a field of nursing
* It combines public health and nursing
* It focus in population and environment factors that may impact to people’s health
* It emphasize in health promotion, illness prevention, and wellness
* It promotes client responsibility and self-care; it uses aggregate measurement and analysis. It use principle of organizational theory
* It involves inter-professional collaboration.

Pubic Health is for:

* The sanitation of environment,
* The control of communicable infections,
* The education of the individual in personal hygiene,
* The organization of medical and nursing services for The early diagnosis and preventive treatment of disease, and
* The development of the social machinery to insure everyone a standard of living adequate for the maintenance of  health

Primary Prevention for Health Promotion:

* Health Education
* Adequate Nutrition

* Adequate working environment and recreation, resting
* Pre-marriage counseling and education
* Genetic Counseling
* Regular Physical Examination

 Primary Prevention for Health Protection:

* Comply with immunization
* Individual hygiene
* Improve environment sanitary
* Occupational Safety
* Prevent Accident in all ages
* Adequate nutrition
* Avoid cancer cause agents
* Avoid allergic

Secondary Prevention:

* Identify cases in the community
* Screen and test
* Conduct special physical examination
* Treat and prevent the progress of illness
* Avoid the spreading of disease
* Reduce the disability time

Tertiary Prevention:

* Treat and stop the disease progress and avoid complication and side effect
* Limit the chance for disability
* Provide rehabilitation for physical and psychological well-being, occupational therapy, availability of long-term care
